Leadership decision
Decide which measurement capabilities leadership will repair, redesign, consolidate, retire, validate, defer or stop; who owns the next evidence gate; and which measures remain unsafe for material decisions until that gate passes.

Finance, operations, marketing and customer teams can report different results while each follows its own rules correctly. Reconcile those definitions and sources so leadership knows which evidence it can rely on for the decision ahead.

The Measurement and Decision Intelligence Audit is for that condition. It follows a bounded management decision through six connected layers, collection, quality, identity, governance, reporting and decision use, then states what can be trusted, for which decision, within which limits and what must be repaired or stopped. It starts with the decision and market connection, not a preferred analytics platform.

This is an Abrasim decision-support working methodology. It is not a financial, statutory, internal-audit or assurance engagement; it does not certify compliance, privacy, security, controls or the completeness of any population. Qualified and authorised owners must make those conclusions. Technical observations, sampled tests and reconciliations remain bounded by their recorded source, period, population, method and uncertainty.

Executive decision card: complete before fieldwork

Leadership controlMinimum entry
Decision to makeOne dated question naming what leadership may fund, repair, redesign, consolidate, retire, validate, defer or stop.
Market connectionThe customer, user, service recipient, partner or market outcome that the decision is intended to improve or protect.
Decision evidenceThe measures, operational records, definitions and acceptable confidence needed to make the decision.
AuthorityNamed economic buyer, decision owner, evidence-access authority, information authority, source owners and operating owners.
BoundariesSystems, reports, decisions, populations, periods and locations in scope; legal, assurance, architecture and implementation work excluded.
Stop routeStop or escalate for unlawful or unsafe access, an unowned decision, uncontrolled personal or sensitive information, an unresolvable source conflict, materially insufficient evidence or no capacity to act.
Required outputsApproved decision inventory, lineage-and-quality register, metric-and-report register, and remediation decision register.

1. The Market Connection Context

The buyer recognises the problem when reports consume substantial attention but do not settle the decisions that affect customers and operating value. Meetings drift into whose number is right. Teams export more data. Dashboards multiply. The delayed decision, however, remains the same: which demand to serve, which customer journey to repair, which capacity to allocate, which investment to continue or which intervention to stop.

Typical signals include:

  • customer acquisition appears healthy while qualified, fulfilled or retained outcomes do not reconcile;
  • finance, commercial, product and operations teams use different definitions for the same material measure;
  • a report presents totals without its population, exclusions, period, source or calculation version;
  • customer behaviour crosses domains, devices, channels or offline processes, but continuity is either missing or pursued without a proportionate privacy basis;
  • executive packs describe what happened but have no threshold, action, decision owner or record of what happened next;
  • tracking changes, manual adjustments or source-system changes reach reports without controlled approval or a visible definition change;
  • an automation, transformation or product investment lacks a defensible baseline against which its contribution can later be assessed;
  • a material result can be reproduced only through one analyst's undocumented intervention.

The primary value path is:

Customer and operating signals
→ stable definitions and controlled evidence
→ explanation within known limits
→ accountable decision
→ owned action
→ observed market and operating learning

The audit protects the connection between the organisation and the markets it serves. A collection defect can hide a customer need. A definition dispute can delay a service decision. A report without an action threshold can consume capacity without changing customer value. These may affect revenue, margin, cost, capacity, retention, risk, investment efficiency or service value, but every economic effect is REQUIRES VALIDATION until the organisation supplies a defensible baseline, scope, source and causal logic.

The six connected audit layers

LayerDecision questionMinimum evidenceTrust limit to expose
CollectionIs the material event or state captured at the point where it occurs?Source configuration, event or transaction record, change history and observed testA firing tag is not proof that the recorded event represents the intended business outcome.
QualityIs the record complete, valid, consistent, unique, timely and reconcilable enough for the decision?Defined population, test method, exceptions, comparison source and retained resultA sample does not establish total-population assurance.
IdentityCan the relevant journey or entity be connected responsibly across approved sources?Identifier rules, matching method, lawful purpose, consent or authority, and unmatched populationMore linkage is not automatically more useful or proportionate.
GovernanceAre definitions, access, changes, incidents and retention owned and controlled?Decision rights, approvals, access records, change log, retention rule and escalation routeA documented policy is not evidence that controls operate.
ReportingDoes each material report preserve definition, context, uncertainty and decision hierarchy?Report inventory, semantic or calculation logic, refresh evidence, usage and ownerVisual consistency is not semantic consistency.
Decision useDoes an authorised owner act when an evidenced threshold or question requires it?Decision record, forum, threshold, action, owner, date and later reviewCorrelation and attributed credit do not establish causal impact.

The causal chain for each material finding is:

Recognisable decision or market condition
→ break in collection, quality, identity, governance, reporting or use
→ customer or operating consequence
→ potential economic exposure
→ evidence needed to quantify and explain it
→ dated leadership disposition

The decision this audit must serve

The audit must end with a bounded decision: which measurement capabilities leadership will repair, redesign, consolidate, retire, validate, defer or stop; who owns the next evidence gate; and which measures remain unsafe for material decisions until that gate passes.

The method is appropriate when the decision crosses functions or source systems, measurement disputes delay action, a material investment needs a baseline, or the consequences of a wrong signal justify independent reconciliation. It is also appropriate after significant journey, platform, CRM, reporting or operating-model change when leadership needs to re-establish what remains comparable.

Do not use the method to justify a dashboard or tool already selected, to claim total assurance from limited tests, to expand personal-data collection without a lawful and proportionate purpose, or to replace a statutory, internal-audit, privacy, security, actuarial, accounting or legal review. A narrow configuration check is sufficient when one known defect, one accountable owner and one acceptance test fully contain the decision. Stop rather than broaden the audit when no named owner can act on the result.

Scope, exclusions and dependencies

The minimum useful scope names:

  • the management decisions and decision dates being served;
  • the affected customer, user, market or operating populations;
  • the systems, data products, reports, interfaces and manual adjustments in the path;
  • the periods, definitions, versions and source owners to be tested;
  • the material reports and recurring decision forums;
  • the access, privacy, security, retention and evidence-handling constraints;
  • the implementation, assurance and legal conclusions explicitly excluded.

Dependencies include executive authority to resolve disputed definitions, controlled access to authoritative sources, source and report owners who can explain transformations, a suitable comparison record, and protected time for leadership to decide. If these conditions do not exist, the first output is a visible dependency and validation plan, not a confident maturity label.

Expected results are operational artefacts and decisions: a decision inventory, a source-to-decision lineage map, bounded quality findings, metric contracts, report dispositions, governance ownership, a sequenced remediation decision and a review cadence. No result in this guide guarantees improved commercial performance.

2. The Leadership vs. Delegation Boundary

The audit can be led internally or externally. The deciding question is whether the team has sufficient cross-functional authority, controlled access, independent challenge and protected capacity, not whether it possesses one particular analytics tool.

BoundaryRequired responsibility
Executive sponsor ownsThe market or business outcome, decision question, scope, proportionality, participating authority, budget boundary, conflicts, decision date and acceptance of residual risk.
Specialist may leadDecision interviews, evidence requests, source-to-report tracing, configuration observation, quality test execution, metric-contract drafting, report-use analysis, finding synthesis and facilitation.
Shared responsibilityDefinitions, source interpretation, sampling, lawful-access design, evidence classification, root-cause challenge, remediation options, acceptance tests, handover and review design.
Must not be outsourcedFinal business decision, risk acceptance, legal/privacy/security authority, source-system accuracy attestation, customer or employee accountability, access approval and operating ownership.

When an internal team can lead

A capable internal team can run the audit when it has a sponsor able to resolve cross-functional disputes, source owners who can provide controlled evidence, an independent challenger who did not design every material component, explicit information authority and enough capacity to retain a complete audit trail. Independence can be achieved through governance and challenge; it does not automatically require an external firm.

When an independent senior partner is justified

Independent leadership is proportionate when definitions affect several budgets, teams dispute the source of truth, previous implementation decisions require challenge, access spans several custodians, a material investment depends on the conclusion or internal incentives make self-assessment unreliable. The partner should leave definitions, evidence, decisions and operating ownership with the organisation.

An Abrasim Measurement and Decision Intelligence Assessment is the paid assessment path when the work crosses collection, business systems, definitions, privacy boundaries, reporting and executive decision behaviour. The engagement should begin with the bounded decision and evidence gate, not a platform sale. Transformation, implementation, operating support or scale work follows only if leadership accepts the diagnosis and separately funds the next gate.

When a narrower specialist or simpler intervention is sufficient

Use a platform specialist when the business definition and decision are settled and the remaining task is one bounded configuration defect with an observable acceptance test. Use a data engineer when lineage and transformation logic are already governed but a defined pipeline must be repaired. Use a privacy or security specialist for qualified conclusions in those domains. Retire an unused report without a broad audit when its owner, dependencies and control obligations are known.

Stop and escalation conditions

Stop evidence collection and escalate to the named authority when access lacks a valid purpose, secrets or personal information would be copied into an uncontrolled workspace, a safety or security incident is discovered, an assurance conclusion is being implied, a source owner contests the meaning of a material record, or the sponsor cannot protect a decision date. Pause a finding when the tested population or period cannot support the proposed claim. Stop a remediation option when it has no decision owner, no measurable acceptance gate or a burden greater than the consequence it addresses.

Escalation follows the nature of the issue: source accuracy to the source owner; definition conflict to the business decision owner; access and retention to the information authority; privacy, security, legal or regulatory interpretation to the qualified authority; unresolved commercial trade-offs to the economic buyer; and scope or independence conflict to the executive sponsor.

Questions for evaluating a provider

Ask a prospective provider:

  1. Which named decisions will determine the scope before you recommend a platform or dashboard?
  2. How will you distinguish an observed configuration fact, a reconciled quality result, an interpretation and an untested assumption?
  3. How will you trace one executive measure back through definitions, transformations, collection and source ownership?
  4. What sample, population, period, tolerance and comparison method will each quality conclusion use?
  5. How will you prevent your team from copying sensitive evidence into unapproved tools or locations?
  6. How will you test report use and decision behaviour rather than report appearance alone?
  7. Which conclusions require our finance, privacy, security, legal, internal-audit or source owners rather than your sign-off?
  8. What will you recommend we retire, stop or leave unchanged?
  9. Which artefacts and operating controls will remain usable by our team after handover?
  10. How will dissent, source conflict, uncertainty and rejected options appear in the executive record?

Reject a proposal that begins with licence selection, promises a universal source of truth without defining decision-specific limits, equates more tracking with better intelligence, or asks the same delivery team to self-certify assurance.

3. The Step-by-Step Execution Standard

Use the Measurement Audit Charter and Decision Inventory, Source, Lineage and Quality Register, Metric, Report and Governance Register, and Finding, Remediation and Decision Register as controlled working files. Assign a custodian, version and approved storage location before use.

Step 01: Charter the decision and trust question

Purpose and decision served
Convert a broad concern such as “we do not trust the data” into one decision, affected market connection and explicit evidence threshold. The charter prevents a platform inventory from replacing the leadership question.

Accountable owner
Executive sponsor, with the economic buyer accepting scope and proportionality.

Delegated lead and contributors
Audit lead; decision owner; finance, commercial, operations, product and data representatives; information authority; evidence coordinator.

Preconditions, inputs and access
A real upcoming decision, its forum and date; the suspected customer or operating consequence; known reports or measures; authority to invite affected owners; approved working environment.

Actions

  1. Write the decision as a dated choice: fund, repair, redesign, consolidate, retire, validate, defer or stop a bounded measurement capability or its dependent intervention.
  2. Name the external market, customer, user or service outcome the decision is intended to improve or protect.
  3. Record the measures leadership currently uses, why confidence is contested and the consequence of deciding late or incorrectly without assigning an unverified monetary value.
  4. Define systems, decisions, reports, populations, periods and locations in scope, and list implementation, assurance, legal and compliance conclusions out of scope.
  5. State the minimum evidence, tolerance, uncertainty and human approvals required for the decision.
  6. Set the evidence-close date, executive decision date, burden limit and stop conditions.

Evidence to retain
Approved problem statement, meeting or decision reference, affected value path, known source references, scope rationale, exclusions, authority and version history.

Decision or scoring rule
Use proceed only when one authorised decision, one decision date, a bounded scope and minimum evidence are explicit. Use change when the scope exceeds the decision. Use defer when authority is not available. Use stop when no owner can act.

Output and template
Completed charter rows in the Measurement Audit Charter and Decision Inventory, with stable A3-AUD-* and A3-DEC-* identifiers.

Acceptance gate
PROCEED only if the sponsor and decision owner accept the question, scope, exclusions, evidence threshold, dates and stop route; otherwise CHANGE, DEFER or STOP.

Leadership checkpoint
The economic buyer confirms that the decision consequence justifies the proposed evidence burden and that no technology purchase is presupposed.

Human control point
Only the authorised sponsor may accept the charter or widen the decision; an analyst must not infer authority from access to systems.

Failure modes and safeguards
Failure modes are an audit of everything, a hidden dashboard brief, an invented value claim or a scope that cannot finish before the decision. Safeguard with one decision card, explicit exclusions and a change record for every expansion.

Change, stop or escalation condition
Escalate competing decision questions to the sponsor. Stop if the decision is performative, already irrevocably made or has no accountable owner.

Handoff to the next step
Pass the accepted charter and decision IDs to Step 02 for roles, independence, information authority and escalation design.

Step 02: Assign authority, ownership and independent challenge

Purpose and decision served
Establish who can supply evidence, challenge meaning, approve controlled access, accept findings and make the final decision.

Accountable owner
Executive sponsor.

Delegated lead and contributors
Audit lead; decision owner; source-system owners; business metric owners; technical owners; information authority; privacy, security, legal or internal-audit specialists where applicable; independent challenger.

Preconditions, inputs and access
Accepted charter, organisational decision rights, system ownership list, applicable policies and declared delivery or supplier interests.

Actions

  1. Assign the executive sponsor, economic buyer, decision owner, audit lead, evidence coordinator, information authority and working-pack custodian.
  2. Map a business owner and technical owner to each initial decision-critical measure and source.
  3. Name who can approve access, sampling, extraction, retention, definition changes, report retirement, remediation funding and residual risk.
  4. Identify people who designed or operate the audited components and record conflicts or self-review risk.
  5. Appoint an independent challenger with access to evidence and freedom to record dissent; define recusal or additional review where independence is insufficient.
  6. Publish escalation routes and response dates for definition, access, incident, evidence and commercial disputes.

Evidence to retain
Role acceptance, authority references, conflict declarations, recusal decisions, escalation route, controlled contact list and meeting quorum.

Decision or scoring rule
A role is assigned only when the named person accepts it and possesses the authority stated. A title or distribution-list entry is not acceptance.

Output and template
Ownership and authority records in the Measurement Audit Charter and Decision Inventory.

Acceptance gate
PROCEED when every material decision, source, definition, access route and final disposition has a named accepted owner and independent challenge is credible; otherwise CHANGE or DEFER.

Leadership checkpoint
The sponsor resolves gaps that the audit lead cannot solve, including cross-functional source ownership and the authority to retire a report.

Human control point
Information, privacy, security, legal and risk authorities retain their mandates; provider or analytics staff cannot approve their own access or assurance.

Failure modes and safeguards
Failure modes are generic ownership, one person holding incompatible roles and late discovery that nobody can accept a definition. Safeguard with named role acceptance, conflict disclosure and quorum before fieldwork.

Change, stop or escalation condition
Pause affected evidence work for missing access authority or unmanaged conflict. Stop if leadership will not assign decision ownership.

Handoff to the next step
Give the accepted role map to Step 03 so interviews and inventory requests reach people who make and operate the decisions.

Step 03: Inventory decisions before metrics

Purpose and decision served
Identify the recurring choices measurement must support before evaluating events, dashboards or platforms.

Accountable owner
Decision owner for each inventoried decision; executive sponsor for portfolio scope.

Delegated lead and contributors
Audit lead; executive and operating decision-makers; finance partner; customer, commercial, service, product and operational owners; report owners.

Preconditions, inputs and access
Accepted role map; calendar and terms of reference for relevant forums; current packs and dashboards; list of planned investments and interventions.

Actions

  1. Interview decision-makers using concrete recent examples, asking what they decided, when, under which authority and what happened next.
  2. Record each decision's question, outcome served, affected population, cadence, deadline, consequence and authorised forum.
  3. Identify the minimum measures, context, thresholds, comparison and explanatory evidence expected before action.
  4. Link every recurring report and metric to a decision ID; mark “visibility” as incomplete until it has a named user and action.
  5. Record decisions made without reports and reports that serve no evidenced decision.
  6. Rank the inventory by material consequence and the chartered decision, not by seniority, metric volume or dashboard prominence.
  7. Select a bounded set for end-to-end tracing and state why excluded decisions are not material to this audit.

Evidence to retain
Interview notes with consent, decision-forum records, report references, decision frequency, threshold statements, disagreement and selection rationale.

Decision or scoring rule
A decision is traceable only if it has an owner, forum or authority, timing, outcome, affected population, expected action and evidence need. Otherwise classify it requires clarification or remove it from scope.

Output and template
Decision and report-inventory rows in the Measurement Audit Charter and Decision Inventory.

Acceptance gate
PROCEED when every in-scope metric or report has a decision relationship or an explicit retirement-review status, and the sponsor accepts the tracing sample.

Leadership checkpoint
Decision owners confirm that the inventory represents actual choices rather than an idealised reporting process.

Human control point
Only the decision owner may confirm the consequence and action; report metadata must not be treated as evidence of use.

Failure modes and safeguards
Failure modes are inventorying every metric, accepting “monitor” as an action and interviewing report producers only. Safeguard with recent decision examples, affected-market questions and decision-first sampling.

Change, stop or escalation condition
Change the scope if the selected measures do not serve the chartered decision. Escalate conflicting decision ownership. Stop tracing a report with no owner or decision and submit it for retirement review.

Handoff to the next step
Pass selected decision, metric, report and population IDs to Step 04 to design evidence access and tests proportionate to their consequence.

Step 04: Design the evidence, access and test plan

Purpose and decision served
Create a controlled, proportionate route to evidence before anyone extracts data or enters a system.

Accountable owner
Information authority for access and handling; audit lead for method; decision owner for proportionality.

Delegated lead and contributors
Evidence coordinator; source owners; data and analytics specialists; privacy and security owners; finance or operational comparison-source owners; independent challenger.

Preconditions, inputs and access
Selected decision inventory, source register, applicable access and retention rules, approved environment, known classifications and incident route.

Actions

  1. For each decision, state the evidence question and the minimum source, fields, period, population, grain and comparison needed.
  2. Record lawful purpose or organisational authority, information classification, approved users, access location, extraction limits, retention, disposal and incident route.
  3. Prefer read-only, source-controlled access; prohibit secrets, access tokens and unnecessary personal information in the working pack.
  4. Design tests for completeness, validity, consistency, uniqueness, timeliness and reconciliation only where they serve a stated decision.
  5. Specify population, denominator, exclusions, sampling method, edge cases, comparison source, tolerance and known limitations before testing.
  6. Define evidence IDs, chain-of-custody or source-version references and how transformations or manual adjustments will be reproduced.
  7. Obtain named approvals before access and record denied or unavailable evidence as a gap rather than silently substituting a weaker source.

Evidence to retain
Evidence request, approvals, access log, data classification, test specification, sample rationale, source owner, version, retention date and denied-access record.

Decision or scoring rule
An evidence route is approved only when purpose, authority, source, population, handling, authorised users, retention and incident response are explicit. Otherwise use change, defer or stop.

Output and template
Access and test-plan rows in the Source, Lineage and Quality Register.

Acceptance gate
PROCEED only for approved routes whose test design can support the intended claim; all other evidence remains REQUIRES VALIDATION or is excluded.

Leadership checkpoint
The decision owner confirms that evidence burden and sample design are proportionate to the consequence; the information authority confirms access controls.

Human control point
Qualified human authorities decide access, privacy, security, retention and legal interpretation. Automated discovery does not grant permission to inspect or copy a source.

Failure modes and safeguards
Failure modes are uncontrolled exports, retrospective sampling rules, collecting every available field and confusing access with authority to reuse. Safeguard with pre-registered tests, least-privilege access and one material evidence item per register row.

Change, stop or escalation condition
Stop immediately for unsafe credentials, unapproved sensitive information or an incident. Escalate a denied material source to the decision owner; reduce the claim rather than bypass the control.

Handoff to the next step
Provide approved evidence routes, test IDs and source owners to Step 05 for source-to-decision lineage tracing.

Step 05: Trace collection, identity and lineage end to end

Purpose and decision served
Show how a material real-world event or state becomes the measure a decision-maker sees, including every point where meaning, population or value can change.

Accountable owner
Business metric owner, supported by the technical source and transformation owners.

Delegated lead and contributors
Data analyst or engineer; analytics implementation specialist; application and integration owners; CRM or operational-system owner; report owner; information authority.

Preconditions, inputs and access
Approved access plan; decision, measure, report and source IDs; current configuration and code or transformation versions; representative records and environments.

Actions

  1. Start with the value displayed in the decision forum and record its label, definition, period, filters, population, refresh and calculation version.
  2. Trace backwards through report, semantic model, query or calculation, transformation, integration, collection and originating business event or system record.
  3. Record joins, identity keys, consent or authority conditions, matching rules, unmatched records, cross-domain or offline imports and any manual intervention.
  4. Observe representative event or transaction paths in an authorised environment; distinguish configured intent from witnessed behaviour.
  5. Mark every change of grain, time zone, currency, status, deduplication rule, attribution model, default filter and exclusion.
  6. Link configuration, query, schema, definition and change-history evidence to the relevant lineage node without copying protected values unnecessarily.
  7. Record missing, inaccessible, undocumented or ownerless lineage as an evidence gap; never draw an inferred connection as verified.

Evidence to retain
Source and node IDs, controlled configuration or query references, version, observed test record, transformation steps, join and identity rules, owners, limitations and public claim status.

Decision or scoring rule
A lineage path is traceable only when every decision-material transformation has an owner, reproducible reference and effect on meaning recorded. Use partial for visible gaps and untraceable when the displayed value cannot be connected to an authoritative event or record.

Output and template
Lineage-node and evidence rows in the Source, Lineage and Quality Register.

Acceptance gate
PROCEED TO TEST when the lineage is sufficiently traceable to define the population and reproduce the measure; otherwise VALIDATE, CHANGE SOURCE or DO NOT USE FOR THE DECISION.

Leadership checkpoint
Business and technical owners jointly confirm what the measure means and where its known discontinuities sit.

Human control point
The business owner accepts business meaning; the information authority accepts identity use; neither acceptance may be inferred from technical documentation alone.

Failure modes and safeguards
Failure modes are architecture diagrams without source evidence, assuming vendor interfaces expose raw truth, and treating unmatched identity as zero. Safeguard with record-level trace samples, explicit node IDs and visible unknown or unmatched populations.

Change, stop or escalation condition
Escalate a material identity or definition conflict. Stop the lineage claim where access authority ends, and stop use of a measure for the chartered decision if critical transformations cannot be reproduced.

Handoff to the next step
Give traceable lineage paths, testable populations and gaps to Step 06 for bounded quality testing and reconciliation.

Step 06: Test quality and reconcile operational truth

Purpose and decision served
Establish whether each material measure is fit enough for its named decision and explain differences between sources without demanding artificial equality.

Accountable owner
Business metric owner; source owners accept source-specific findings.

Delegated lead and contributors
Audit analyst; data or analytics engineer; finance, CRM, ecommerce, service or operational comparison-source owners; independent challenger.

Preconditions, inputs and access
Approved test specification, traceable lineage, controlled source access, frozen period or version, comparison definitions and incident route.

Actions

  1. Execute the pre-registered completeness, validity, consistency, uniqueness and timeliness tests against the stated population or sample.
  2. Reconcile decision-material digital or analytical records with an appropriate operational source such as booked transactions, fulfilled orders, qualified opportunities, service cases or backend accounts.
  3. Normalise period, time zone, status, population, currency, exclusions and late-arriving records before interpreting variance.
  4. Separate expected differences, such as attribution or identity rules, from defects, unknown variance and unavailable evidence.
  5. Re-run representative edge cases and retain the query, method, result, exception population and source versions.
  6. Ask source owners to corroborate or contest the observation and record dissent without overwriting the original result.
  7. Classify decision fitness as fit within stated limits, fit only for directional or diagnostic use, requires validation or not fit for this decision.

Evidence to retain
Test ID, decision and measure IDs, source versions, population, denominator, exclusions, method, expected and observed result, exceptions, tolerance basis, reconciliation, owner response and uncertainty.

Decision or scoring rule
Never use an invented universal tolerance. Apply the approved decision-specific acceptance rule. Where no authorised tolerance exists, record baseline required and REQUIRES VALIDATION; do not relabel unexplained variance as acceptable.

Output and template
Quality-test and reconciliation rows in the Source, Lineage and Quality Register.

Acceptance gate
Each tested measure receives one explicit fitness disposition and evidence reference; no sampled test is presented as total assurance.

Leadership checkpoint
The decision owner accepts whether the stated fitness and uncertainty are sufficient for the decision, or directs further validation.

Human control point
Source owners review source meaning, and authorised incident owners handle suspected control or security failures. The audit lead cannot self-approve an unexplained variance.

Failure modes and safeguards
Failure modes are comparing unlike definitions, hiding exclusions, testing an easy period only and treating close totals as proof of correct records. Safeguard with pre-registered rules, multiple dimensions or edge cases where material, and retained exception evidence.

Change, stop or escalation condition
Change the test if the agreed population cannot be reproduced. Escalate material unexplained variance or suspected incident. Stop using a measure where the decision consequence exceeds its evidenced fitness.

Handoff to the next step
Pass verified definitions, bounded fitness dispositions, conflicts and gaps to Step 07 for metric contracts.

Step 07: Contract decision-critical measures

Purpose and decision served
Turn contested or implicit metrics into versioned agreements about meaning, calculation, ownership, limits and response.

Accountable owner
Business metric owner, with the decision owner approving material use.

Delegated lead and contributors
Analytics lead; technical metric owner; finance, commercial, operations or product owners; source owners; report owner; governance custodian.

Preconditions, inputs and access
Decision inventory, lineage, quality dispositions, competing definitions, source evidence and applicable policy or accounting authority.

Actions

  1. Give each decision-critical measure a stable ID, plain name, business purpose and named decision relationship.
  2. Define formula, unit, grain, numerator, denominator, inclusions, exclusions, period, time basis, currency and treatment of late or corrected records.
  3. Record authoritative source, lineage version, business and technical owner, refresh or latency, baseline period and known limitations.
  4. Reconcile competing definitions; preserve the rejected definitions, affected reports and transition consequences rather than silently replacing them.
  5. Define the threshold or question that triggers action and the guardrail that prevents harmful optimisation.
  6. Classify the measure as learning, leading, operating, commercial outcome or guardrail, and state attribution limits and time lag.
  7. Obtain approval, set an effective date and version, and define change control, impact assessment and communication.

Evidence to retain
Metric contract, source and lineage IDs, definition alternatives, reconciliation record, approval, effective date, affected products, baseline evidence, limitation and version history.

Decision or scoring rule
A metric is contracted only when its definition, source, owner, baseline status, action threshold, guardrail and change route are accepted. Otherwise mark it provisional or do not use for material decision.

Output and template
Metric rows in the Metric, Report and Governance Register.

Acceptance gate
APPROVE only when business and technical owners accept the same version and the decision owner accepts its limits; otherwise VALIDATE, CHANGE or RETIRE.

Leadership checkpoint
The decision owner resolves commercially material definition trade-offs and accepts transition consequences for targets, comparisons or incentives.

Human control point
Finance, legal, privacy, risk or other qualified owners approve definitions within their mandate. A semantic layer cannot settle a business dispute automatically.

Failure modes and safeguards
Failure modes are definitions without exclusions, unversioned spreadsheet logic, one denominator serving unlike populations and technical ownership without business accountability. Safeguard with the full contract, dual ownership and effective-date control.

Change, stop or escalation condition
Escalate irreconcilable definitions to the decision owner. Stop publishing a measure under one label when material audiences still apply incompatible meanings.

Handoff to the next step
Pass approved and provisional contracts to Step 08 to test whether reports use them and actually support decisions.

Step 08: Audit reports as decision products

Purpose and decision served
Determine whether each material dashboard, pack or alert delivers the definition, context, explanation and action needed by a named user.

Accountable owner
Business report owner.

Delegated lead and contributors
Decision-product analyst or designer; report developer; metric owners; decision users; accessibility and content reviewers; audit lead.

Preconditions, inputs and access
Decision inventory, metric contracts, report inventory, access to current and prior versions, refresh evidence, usage evidence and decision records.

Actions

  1. Link each report to its decision, audience, forum, cadence, measure versions and accountable owner.
  2. Test whether outcome, comparison, period, population, definition, source, freshness, uncertainty, threshold and requested action are visible at the point of use.
  3. Compare report totals and filters with contracted measures and source-level test results.
  4. Observe a real or tabletop decision forum and record which questions the report answers, which require manual analysis and what action follows.
  5. Review accessibility, export behaviour, small-screen use where relevant and whether critical meaning depends only on colour or visual position.
  6. Record usage, duplicate products, manual burden, stale or orphaned ownership, and evidence of decisions made outside the report.
  7. Assign retain, redesign, consolidate, retire or requires validation, with acceptance criteria and dependencies.

Evidence to retain
Report ID and version, owner, audience, decision link, measure IDs, screenshots or controlled references, refresh result, observed review notes, accessibility findings, usage evidence and disposition.

Decision or scoring rule
A report is decision-fit only when a named user can interpret its contracted measures and make the intended decision within stated limits. Visual polish, traffic or distribution do not establish fitness.

Output and template
Report rows in the Metric, Report and Governance Register.

Acceptance gate
Every material report has an authorised retain, redesign, consolidate, retire or requires validation disposition, an owner and a dated gate.

Leadership checkpoint
The decision owner accepts the report portfolio disposition and protects the authority to retire duplicated or unused reporting work.

Human control point
Actual decision users validate usability and action; telemetry cannot establish that a viewed report was understood or used responsibly.

Failure modes and safeguards
Failure modes are redesigning visuals before fixing definitions, preserving reports because they are familiar, and equating logins with adoption. Safeguard with decision observation, metric-contract checks and an explicit retirement route.

Change, stop or escalation condition
Stop redesign when underlying measures are not fit. Escalate reports whose retirement affects a control, statutory duty or contractual obligation to the qualified owner.

Handoff to the next step
Pass report dispositions, decision-use evidence and operating exceptions to Step 09 for governance and root-cause diagnosis.

Step 09: Diagnose the operating model and material findings

Purpose and decision served
Explain why measurement trust breaks and whether the cause is collection, quality, identity, governance, reporting, capability, incentives or decision behaviour.

Accountable owner
Executive sponsor for cross-functional findings; relevant operating owner for each finding.

Delegated lead and contributors
Audit lead; independent challenger; metric, source and report owners; data governance, privacy and security representatives; decision users.

Preconditions, inputs and access
Retained inventory, lineage, test, metric, report and forum evidence; ownership and change records; dissent and evidence gaps.

Actions

  1. Group observations by affected decision and audit layer, not by system or supplier alone.
  2. Write each finding as condition, criterion or accepted expectation, retained evidence, customer or operating consequence, likely cause, uncertainty and required decision.
  3. Test alternative explanations such as definition mismatch, expected attribution difference, source timing, process behaviour, capacity constraint and incentive conflict.
  4. Trace governance: who approves definitions, access, changes, incidents and report retirement, and whether those controls demonstrably operate.
  5. Separate observed facts, corroborated interpretations, single-source statements, assumptions, evidence gaps and constraints.
  6. Assign public claim status VERIFIED, INFERRED, PROPOSED or REQUIRES VALIDATION to every material statement intended for wider use.
  7. Review each finding with affected owners and retain confirmation, qualification or dissent without averaging disagreement into a false consensus.

Evidence to retain
Finding ID, linked evidence IDs, affected decision and value path, condition, expectation, consequence, cause alternatives, classification, confidence limits, owner responses and dissent.

Decision or scoring rule
A finding is decision-grade only when its condition is traceable, its consequence is bounded, alternatives were challenged, uncertainty is visible and a named owner can act. Severity without that chain is provisional.

Output and template
Finding rows in the Finding, Remediation and Decision Register.

Acceptance gate
Affected owners accept, qualify or formally dissent from every material finding, and the independent challenger confirms that counter-explanations and evidence gaps remain visible.

Leadership checkpoint
The sponsor decides whether disputed but consequential findings require more evidence, a provisional control response or removal from the decision set.

Human control point
Qualified domain owners decide whether observations imply privacy, security, financial, legal or regulatory consequences. The audit records referral; it does not issue those opinions.

Failure modes and safeguards
Failure modes are tool-centric root cause, maturity theatre, severity based on intuition and erasing dissent. Safeguard with evidence IDs, alternative-cause review, decision-specific consequence and open reviewer disposition.

Change, stop or escalation condition
Downgrade or withdraw a finding when evidence does not support its stated consequence. Escalate urgent control or incident issues through the established route rather than waiting for the final report.

Handoff to the next step
Pass decision-grade findings, provisional findings and open gaps to Step 10 for options and prioritisation.

Step 10: Develop and prioritise proportionate alternatives

Purpose and decision served
Compare ways to improve decision fitness without presuming more tracking, a new platform or a large implementation.

Accountable owner
Decision owner, with the economic buyer owning material funding trade-offs.

Delegated lead and contributors
Audit lead; operating and technical owners; finance partner; privacy, security, risk and procurement contributors as relevant; independent challenger.

Preconditions, inputs and access
Accepted findings, constraints, existing commitments, available capacity, architecture and policy dependencies, and known contractual or retirement obligations.

Actions

  1. Create at least the applicable options: leave unchanged with accepted risk, stop or retire, simplify process or definition, repair the current implementation, redesign governance or reporting, build or buy, and run a bounded validation.
  2. State each option's causal logic, affected decision, customer or operating value, cost basis, capacity burden, dependencies, reversibility and exit route.
  3. Record evidence strength and unknowns; do not fabricate impact, effort, return, target or implementation timing.
  4. Test data minimisation, accessibility, security, privacy, control and human-accountability consequences.
  5. Sequence prerequisite control, definition and source repairs before visual redesign or automation.
  6. Prioritise by decision consequence, evidence strength, feasibility, risk, time to learning, dependency readiness and operating fit using documented rationale rather than automatic arithmetic.
  7. Preserve dissent, rejected options and opportunity cost.

Evidence to retain
Option ID, linked findings and evidence, assumptions, causal logic, cost basis, burden, risk, dependencies, reversibility, option review, rationale, dissent and proposed disposition.

Decision or scoring rule
Use ratings only as structured prompts. A critical control failure can override a high value score, and no added total may automatically select a winner. Dispositions are proceed, validate, change, defer or stop.

Output and template
Option and prioritisation rows in the Finding, Remediation and Decision Register.

Acceptance gate
Every recommended option traces to findings and evidence, includes a no-action or stop comparison where applicable, and has an owner, dependency, acceptance test and exit route.

Leadership checkpoint
The economic buyer reviews displaced work, operating burden and evidence strength before an option enters planning.

Human control point
Only authorised leadership may accept residual risk, displace budget or retire a decision-critical capability.

Failure modes and safeguards
Failure modes are solution anchoring, vendor-led requirements, false precision and prioritisation by score alone. Safeguard with option classes, causal assumptions, ranges only when sourced and narrative decision rationale.

Change, stop or escalation condition
Change an option when a prerequisite or guardrail cannot pass. Stop options with no owner, no feasible acceptance test, no proportionate purpose or no operating capacity.

Handoff to the next step
Pass viable and rejected options with their rationale to Step 11 for a controlled proof or implementation plan.

Step 11: Design the controlled remediation or proof plan

Purpose and decision served
Turn a preferred option into a bounded, reversible next move that can produce decision evidence before wider investment.

Accountable owner
Delivery owner, with the benefit or outcome owner accepting the measurement logic.

Delegated lead and contributors
Implementation lead; business and technical metric owners; source and report owners; change, privacy, security and accessibility specialists; finance partner.

Preconditions, inputs and access
Prioritised option, accepted findings, architecture and control constraints, available capacity, metric contracts, baseline status and procurement route where relevant.

Actions

  1. Define the smallest scope that can test the causal assumption or complete a necessary control repair.
  2. Name the target population, environment, comparison method, evidence window and limitations; use baseline required where no defensible baseline exists.
  3. Specify deliverables, accountable owners, dependencies, acceptance tests, guardrails, evidence retention and rollback, containment or exit route.
  4. Create an agreed dated plan using evidence and decision gates rather than unsupported elapsed-time promises.
  5. Separate learning, delivery, operating adoption, commercial outcome and guardrail measures.
  6. Confirm access, privacy, security, accessibility, change and service-continuity controls before implementation.
  7. Define the handover artefacts, capability transfer, support ownership, review forum and scale/change/stop authority.

Evidence to retain
Approved scope, option and finding IDs, causal assumption, baseline evidence or gap, plan, resourcing, controls, acceptance tests, measures, guardrails, exit route and approvals.

Decision or scoring rule
The plan is ready for decision only when it can produce or protect evidence fit for the decision within a bounded scope and when guardrails and exit are owned. Otherwise change, validate, defer or stop.

Output and template
Proposed decision, measure and dated-gate rows in the Finding, Remediation and Decision Register.

Acceptance gate
Business, technical, information and delivery owners accept scope, capacity, evidence, controls, acceptance tests and exit before funding approval.

Leadership checkpoint
The economic buyer confirms the evidence sought justifies the investment and that a simpler intervention has not been ignored.

Human control point
Named qualified owners approve control conditions; the delivery partner cannot approve its own success criteria or waive a guardrail.

Failure modes and safeguards
Failure modes are an open-ended roadmap, dates without dependencies, implementation before baseline and success defined as delivery. Safeguard with bounded scope, separate measure records and dated evidence gates.

Change, stop or escalation condition
Return to Step 10 if the option cannot be bounded or operated. Stop if evidence cannot distinguish the intended learning, critical controls fail or the exit route is not credible.

Handoff to the next step
Submit the plan, evidence pack, options and dissent to Step 12 for the authorised executive decision.

Step 12: Make the executive decision and transfer ownership

Purpose and decision served
Record an authorised disposition with its rationale, commitments, controls and operating owners instead of treating delivery of an audit report as completion.

Accountable owner
Named decision owner in the authorised forum.

Delegated lead and contributors
Audit lead presents; economic buyer, benefit owner, delivery owner, metric owners, information authority, finance partner and independent challenger contribute.

Preconditions, inputs and access
Accepted charter, decision-grade findings, option comparison, controlled plan, measurement contracts, open risks, dissent and approval authority.

Actions

  1. Present the decision question, market connection, evidence status, limits, findings, alternatives, burden, dependencies and guardrails.
  2. Ask the independent challenger and affected owners to state unresolved evidence gaps, conflicts and dissent.
  3. Record the authorised disposition: proceed, validate, change, defer or stop; do not translate silence into approval.
  4. Record rationale, rejected options, accepted exposure, resource commitment, risk acceptance and the next dated evidence or decision gate.
  5. Assign benefit, delivery, business metric, technical metric, source, report and governance ownership.
  6. Transfer working files, definitions, lineage, tests, decision history, access obligations, retention actions, runbooks and incident routes.
  7. Obtain explicit handover acceptance and close or reassign provider access.

Evidence to retain
Decision record, meeting authority, disposition, rationale, dissent, owners, commitments, accepted risks, approval references, handover inventory, access closure and next gate.

Decision or scoring rule
Only the authorised forum may issue the disposition. Proceed does not mean the evidence is certain; it means leadership accepts the stated limits, controls and next gate.

Output and template
Decision and handover rows in the Finding, Remediation and Decision Register.

Acceptance gate
CLOSED FOR AUDIT only when the decision and dissent are recorded, each accepted action and measure has an owner, the next gate is dated and handover is accepted; otherwise the audit remains open.

Leadership checkpoint
The decision owner reads back the disposition, accepted uncertainty, commitments and stop authority to the forum.

Human control point
Final choice, risk acceptance, access closure and business accountability remain with named humans; no scoring model or provider makes the decision.

Failure modes and safeguards
Failure modes are report acceptance without a decision, ambiguous action verbs, ownerless remediation and lingering supplier access. Safeguard with separate decision, measure and gate records plus signed handover acceptance.

Change, stop or escalation condition
Escalate decisions made outside authority or without mandatory controls. Stop delivery if the recorded disposition and funded work conflict.

Handoff to the next step
Pass accepted measures, controls, runbooks, ownership and next gate to Step 13 for operating review.

Step 13: Operate, review and decide whether to change, stop or scale

Purpose and decision served
Keep decision evidence trustworthy as sources, definitions, journeys, policies and operating behaviour change, and judge observed contribution within its limits.

Accountable owner
Decision owner and benefit owner; business metric owner for definition and use.

Delegated lead and contributors
Technical metric owner; source and report owners; data steward; operating teams; privacy and security owners; finance or evaluation partner; independent reviewer at material gates.

Preconditions, inputs and access
Authorised decision, accepted handover, operating runbook, metric contracts, quality controls, baseline or baseline plan, guardrails and dated review gate.

Actions

  1. Run scheduled quality, reconciliation, freshness, access, report-use and definition-version checks, retaining results and exceptions.
  2. Record actual measure values with unit, population, period, source version, quality status and comparator; do not overwrite the agreed baseline.
  3. Review decision records and operating action to distinguish report delivery from adoption and adoption from commercial outcome.
  4. Investigate exceptions, incidents, manual burden, definition drift and guardrail breaches; record containment and root-cause status.
  5. Compare results with the approved threshold while stating attribution limits, confounders, time lag and missing counterfactual evidence.
  6. Decide continue, change, hold, stop or scale; require new evidence and capacity review before extending to a new population or decision.
  7. Update contracts, lineage, runbooks, access, retention and training, and return material scope change to a new charter.

Evidence to retain
Actual results, source and definition versions, quality status, exceptions, actions, guardrail outcomes, adoption evidence, operating burden, causal interpretation, review decision and approval.

Decision or scoring rule
Scale only when the acceptance threshold and guardrails pass for the defined population, evidence is decision-fit, operating ownership is demonstrated and the new context is validated. Stop or change when guardrails fail, burden is unsustainable or the causal assumption is not supported.

Output and template
Review measure and decision rows in the Finding, Remediation and Decision Register, with updated metric and governance records where definitions or controls change.

Acceptance gate
Each review ends with a named disposition, evidence status, owner and next date; monitor alone is not an accepted disposition.

Leadership checkpoint
The decision owner accepts causal limits and opportunity cost before continuing or scaling investment.

Human control point
Qualified authorities assess incidents and guardrails; the economic buyer accepts resource and risk trade-offs; affected customer or employee accountability is never delegated to the analytics system.

Failure modes and safeguards
Failure modes are success declared from implementation, metric drift hidden by a stable label, scaling on attributed results alone and controls decaying after handover. Safeguard with versioned contracts, separate outcome and guardrail measures, retained review records and independent challenge at material gates.

Change, stop or escalation condition
Re-charter material changes in population, purpose, source, identity method or decision. Stop for failed guardrails, withdrawn authority, unmanageable evidence quality or an intervention whose burden no longer justifies its value.

Handoff to the next step
There is no automatic next phase. Continue operating the accepted controls, return to Step 01 when the decision changes, or close the capability with retained evidence and accountable disposal.

4. The Evidence & Measurement Checklist

Copy the applicable tests into the working pack. A tick means the named evidence exists and the owner has accepted the test; it does not mean an analyst believes the condition is probably true.

Preflight and authority

  • One dated leadership decision names what may be funded, repaired, redesigned, consolidated, retired, validated, deferred or stopped. Evidence: approved charter and decision ID. Owner: executive sponsor.
  • The affected customer, market, user or service outcome and primary value path are explicit. Evidence: accepted market-connection statement. Owner: decision owner.
  • Scope names systems, reports, decisions, populations, periods, locations and material exclusions. Evidence: versioned scope record. Owner: audit lead.
  • Economic buyer, decision owner, audit lead, information authority, evidence coordinator and independent challenger have accepted their roles. Evidence: role-acceptance record. Owner: executive sponsor.

  • Denied, missing, conflicting or inaccessible evidence remains visible and is not silently replaced by a weaker source. Evidence: evidence-gap and reconciliation records. Owner: evidence coordinator.

Method execution

  • Decisions were inventoried before metrics and every in-scope measure or report has a named decision relationship. Evidence: decision inventory. Owner: decision owner.
  • Each material measure is traced from decision interface through definition, transformation, integration and collection to an authoritative event or record. Evidence: source-to-decision lineage record. Owner: technical metric owner.
  • Completeness, validity, consistency, uniqueness, timeliness and reconciliation tests were applied only where decision-relevant and retained. Evidence: executed quality-test records. Owner: audit lead.

  • The lineage register shows every decision-material transformation, identity rule, manual adjustment and gap. Evidence: reviewed lineage records. Owner: technical metric owner.
  • The quality register gives each tested measure a decision-fitness disposition within stated limits. Evidence: accepted test and reconciliation records. Owner: business metric owner.
  • Metric contracts define formula, unit, denominator, exclusions, source, owners, baseline, threshold, guardrail and attribution limits. Evidence: approved contract versions. Owner: business metric owner.
  • Every material report has a retain, redesign, consolidate, retire or requires-validation disposition with an owner and dated gate. Evidence: report portfolio decision. Owner: decision owner.

Measurement

  • Learning, delivery, operating adoption, commercial outcome and guardrail measures are stored as separate records. Evidence: measurement register. Owner: benefit owner.
  • Every measure records a source, owner, baseline status, cadence, threshold, guardrail and attribution limits. Evidence: accepted measurement contract. Owner: business metric owner.
  • Actual results retain unit, population, period, source version, data-quality status and comparator without overwriting the baseline. Evidence: review-measure rows. Owner: technical metric owner.
  • A threshold leads to a named action and owner rather than an instruction to monitor. Evidence: decision and action record. Owner: decision owner.
  • Scale decisions require evidence from the defined population plus operating-capacity and guardrail acceptance. Evidence: authorised review disposition. Owner: economic buyer.

Decision and handover

  • The authorised forum records proceed, validate, change, defer or stop rather than relying on silence or report acceptance. Evidence: executive decision record. Owner: decision owner.

  • Provider or temporary access is closed or formally reassigned after handover. Evidence: access review record. Owner: information authority.

Review, change and stop controls

  • Source, definition, quality, access, report-use and decision-use controls have an accepted review cadence. Evidence: governance schedule. Owner: data governance owner.
  • Definition and lineage versions are recorded whenever source, calculation, purpose, population or identity changes. Evidence: change and impact record. Owner: technical metric owner.
  • Exceptions, incidents, guardrail breaches and manual operating burden trigger owned containment and review. Evidence: exception and incident records. Owner: operating owner.
  • Each review ends with continue, change, hold, stop or scale and a dated next gate. Evidence: authorised review decision. Owner: decision owner.
  • Material changes in purpose, population, source or decision return to a new charter rather than inheriting old approval. Evidence: closure record and new charter ID. Owner: executive sponsor.

Topic-specific measurement contract

Targets, tolerances and baselines below are not supplied as benchmarks. The organisation must establish and approve them from its own evidence, consequence and operating context.

Measure A: Decision-grade evidence coverage

FieldRequirement
Outcome or decisionDecide whether the chartered executive question has sufficient evidence for an authorised disposition.
MeasureDecision-grade evidence coverage.
TypeLearning.
DefinitionCount of in-scope decision requirements with an accepted source, definition, fitness disposition and limitation, divided by total in-scope decision requirements; report as count and ratio with excluded requirements listed.
Baselinebaseline required at charter approval.
SourceDecision inventory, source-lineage-quality register and reviewer dispositions.
OwnerDecision owner as business owner; evidence coordinator as technical custodian.
CadenceAt evidence-close, before executive decision and whenever a material source or definition changes.
ThresholdProceed only when the decision owner's approved minimum coverage and every mandatory control requirement pass; otherwise validate, change, defer or stop.
GuardrailNo mandatory privacy, security, legal, finance or source-authority requirement may be converted into a percentage trade-off.
AttributionCoverage describes availability and review status, not truth, causal certainty or the quality of the decision made.

Measure B: Decision-critical reconciliation exception disposition

FieldRequirement
Outcome or decisionDecide which material measures can be used, restricted, repaired or withdrawn from the chartered decision.
MeasureDecision-critical reconciliation exception disposition.
TypeLeading.
DefinitionCount of material reconciliation exceptions with an authorised explained, repair, restrict-use or do-not-use disposition, divided by total material exceptions in the frozen test set; retain counts by disposition and do not hide open exceptions in the ratio.
Baselinebaseline required after the first approved reconciliation run.
SourceExecuted quality tests, comparison-source extracts or references, reconciliation records and owner responses.
OwnerBusiness metric owner; technical metric owner maintains the result.
CadenceAt every audit evidence gate and after a material source, transformation or definition change.
ThresholdThe decision owner sets the maximum permitted open material exceptions for the named decision; use requires validation until approved.
GuardrailAn exception may not be labelled explained without retained evidence and owner acceptance; close numerical totals do not prove record-level accuracy.
AttributionA disposition records control of an exception, not elimination of all error or assurance over the whole population.

Measure C: Decision-product action completion

FieldRequirement
Outcome or decisionDecide whether the retained reporting product changes accountable operating behaviour or should be redesigned, consolidated or retired.
MeasureDecision-product action completion.
TypeOperating.
DefinitionCount of actions triggered by an approved report threshold and completed or explicitly re-decided by the due gate, divided by total actions triggered in the review period; report overdue, stopped and superseded actions separately.
Baselinebaseline required from the first complete review period using the contracted decision and action definition.
SourceDecision forum records, report IDs, action register and operating-owner confirmation.
OwnerDecision owner; report owner maintains links and operating owner confirms completion.
CadenceAt each decision forum and consolidated at the agreed operating review.
ThresholdContinue or redesign according to the decision owner's approved action and timeliness threshold; do not treat viewing or distribution as completion.
GuardrailNo action is counted when it bypasses required human approval, customer protection, service control or incident route.
AttributionCompletion shows follow-through after a report signal; it does not prove the report caused the action or that the action improved outcomes.

Measure D: Named market or operating transition rate

FieldRequirement
Outcome or decisionDecide whether the accepted remediation is associated with improvement in the specific customer or operating transition it was intended to support.
MeasureNamed market or operating transition rate.
TypeCommercial outcome.
DefinitionCount of units reaching the contractually defined next state, divided by eligible units entering the defined starting state, for one named population and period; state unit, denominator, exclusions, lag and any monetary definition separately.
Baselinebaseline required from a pre-change or otherwise approved comparison period using the same definition, or an explicit baseline-establishment plan.
SourceAuthoritative customer, transaction, service or operating system reconciled with the decision measure where relevant.
OwnerBenefit owner as business owner; authoritative-source owner as technical owner; finance validates financial interpretation where applicable.
CadenceAt the natural decision and outcome lag agreed in the metric contract, not merely when a dashboard refreshes.
ThresholdProceed, change, hold, stop or scale only against an approved organisation-specific threshold and after guardrails and evidence quality pass.
GuardrailDo not improve the rate by excluding difficult cases, shifting harm, increasing unwanted contact, weakening consent or degrading service to another population.
AttributionObserved movement may reflect seasonality, mix, market conditions, concurrent work and measurement changes; causal contribution requires a fit-for-purpose comparison design.

Measure E: Material measurement guardrail breach

FieldRequirement
Outcome or decisionDecide whether use of the measurement capability must be contained, changed or stopped.
MeasureMaterial measurement guardrail breach.
TypeGuardrail.
DefinitionCount of confirmed material breaches by approved category and affected population in the review period; report each event, severity basis, containment, owner and resolution rather than combining unlike harms into one score.
Baselinebaseline required from the approved incident, access, quality, accessibility and customer-harm registers.
SourceIncident register, access review, quality exceptions, accessibility review, complaints or other authorised control source.
OwnerRelevant information, privacy, security, risk, accessibility or customer-accountability owner; operating owner coordinates containment.
CadenceImmediate on detection and at every decision or scale gate.
ThresholdAny breach at or above the organisation's approved materiality route triggers containment and qualified review; unresolved mandatory breaches block scale.
GuardrailLow incident counts do not justify broader surveillance, weaker controls or non-reporting; absence of reports is not evidence of absence.
AttributionBreach records depend on detection, reporting and classification quality and may understate unobserved events.

Evidence classifications used in this guide

StatusUse
VERIFIEDDirectly supported by a controlled source, observed test or current cited authoritative source, with scope, period and version visible.
INFERREDA reasoned interpretation from verified evidence, with the reasoning, confidence and competing explanation retained.
PROPOSEDA method, definition, option, threshold, control or action awaiting the relevant organisational approval or test.
REQUIRES VALIDATIONAn unconfirmed baseline, population, tolerance, source relationship, causal claim, commercial exposure or future result.

For engagement evidence, separately classify observed fact, corroborated interpretation, single-source statement, assumption, evidence gap and constraint. One label describes how the team obtained or interpreted the evidence; the public status controls what may be claimed. Neither label is a substitute for source, population, method and limitation.

Sources and proof boundaries

  • Google Analytics: Data collection: VERIFIED official source at the source cut-off. Use it to understand the current product's collection behaviour; it does not verify a particular implementation or business definition.
  • Google Analytics: Consent type: VERIFIED official source at the 27 August 2026 source cut-off. It provides current consent-type terminology and behaviour. The organisation's qualified authority must determine the applicable purpose, consent, privacy and legal requirements.
  • Google Analytics: Updates to Analytics data controls: VERIFIED official source at the source cut-off. It demonstrates why platform-control assumptions need dated review; it is not proof that any current configuration is correct.
  • OECD: Data governance: VERIFIED institutional source at the source cut-off. It supports governance context but does not prescribe the organisation's control model.
  • Protection of Personal Information Act 4 of 2013: VERIFIED official South African statutory source. Application to a particular collection, identity or access design requires the organisation's information officer or qualified legal/privacy reviewer.
  • Abrasim six-layer measurement model and this execution sequence: PROPOSED Abrasim working methodology. It is a decision structure, not evidence of maturity, prevalence, assurance or outcome.

The Leadership Audit Working Pack is the shared governance layer. The Measurement and Decision Intelligence Audit: An Executive Standard for Trustworthy Action files add decision, source, lineage, test, metric, report and remediation detail. Keep stable audit, decision, evidence, finding and option IDs across both layers; keep one material evidence item per evidence row; and store decisions, measures and dated gates as separate rows.

Use the method with AI

A prompt to help you work through this guide

Use this prompt to organise your analysis. Share only evidence you are authorised to provide to that model. A person must remain responsible for the decision, privacy limits and final action.

Copy and adapt

You are helping me apply the Abrasim operating guide: “The Measurement and Decision Intelligence Audit: An Executive Standard for Trustworthy Action”.

Situation: Reports exist, but definitions, quality, lineage, ownership or decision use prevent leadership from acting confidently.
Decision to make: Establish what evidence can support which management decision, within which limits, and make an owned repair, use, restriction, withdrawal, validation, deferral or stop decision.
Scope boundary: A decision-led trace across collection, quality, identity, governance, reporting and decision use for a bounded set of material management decisions, measures, sources, reports and operating controls.

Use only the evidence I provide. Do not invent facts, benchmarks, sources, outcomes, customer research or legal conclusions. Keep VERIFIED, OBSERVED, INFERRED, PROPOSED and UNKNOWN separate. Ask focused questions when the evidence or authority is missing.

Help me work through this method by returning:
1. An evidence table with claim, source, date, confidence and limitation.
2. The strongest competing explanations, including what would disprove each one.
3. Decision options with trade-offs, dependencies and the cost of waiting where evidence supports it.
4. The next smallest evidence-gathering action, its accountable owner and acceptance test.
5. A proceed, change, validate, defer or stop recommendation that a named human decision-maker can review.

Treat the attached Abrasim working pack as a blank working record. Do not fill missing rows with plausible data. Flag privacy/POPIA, security, accessibility, safety and authority issues. Cite primary sources for external claims and show uncertainty. End with the conditions that would make us change course or stop.

Working rule: AI can help organise evidence and expose questions; it cannot establish truth, approve risk, make the business decision or replace qualified legal, privacy, security or accessibility review.

Next decision

Decide which measurement capabilities leadership will repair, redesign, consolidate, retire, validate, defer or stop; who owns the next evidence gate; and which measures remain unsafe for material decisions until that gate passes.

Use the cross-functional assessment when definitions, source systems, reporting and decision behaviour span several owners or carry material investment risk; a capable internal team can run the guide when it has sufficient authority, controlled access, independence and capacity.
